Notes

The Notes feature in Cohort Analytics allows you to view and analyze clinical notes and documentation associated with patients in your cohort. This functionality provides access to Electronic Medical Record (EMR) notes, clinical documentation, and other text-based patient information.

Understanding Notes

Notes provide detailed clinical documentation for patients in your cohort, including physician notes, clinical observations, treatment plans, and other relevant medical information. These notes can be searched, filtered, and analyzed to gain deeper insights into patient care and outcomes.

Key Features

  • Clinical Documentation: Access EMR notes and clinical documentation for cohort patients
  • Text Search: Search notes by keywords, phrases, or patterns
  • Note Filtering: Filter notes by date, type, provider, or other criteria
  • Note Analysis: Analyze note content for insights and patterns
  • Download Patient IDs: Export patient identifiers from Notes for use in downstream workflows

Download Patient IDs

You can download cohort patient identifiers from Notes when you need a file for analysis or handoff to other tools.

Large Patient ID downloads: If there are over 1 million rows, avoid opening the CSV in Excel—row limits can truncate the file. Use Notepad, Notepad++, or another editor or data tool meant for very large text/CSV files.

How to Access Notes

  1. Navigate to the Cohort Analytics section
  2. Select or open a cohort
  3. Click on the "Notes" tab or section
  4. Browse or search through available notes
  5. Use filters to refine your note search
  6. Export or analyze notes as needed

Best Practices

  • Use specific keywords when searching notes for better results
  • Combine note searches with other cohort criteria for comprehensive analysis
  • Review note dates to understand the timeline of clinical documentation
  • Document your note search criteria for reproducibility
